Case Management Services 

 

For many persons who are recently unemployed or facing financial difficulties, navigating the complicated web of social services and benefits programs can be a confusing, daunting, and even an impossible task. Case management services help clients develop individualized plans to address their situation, and then help guide clients through implementing those plans and accessing services. Case management services are an integral component of all Community Action Agency community service programs.

Case managers meet with families to help families identify any obstacles to their self-sufficiency and to help them access community resources that will help them overcome their challenges. By partnering with other social service agencies, we maximize resources without duplicating effort.

Through Case Management, Families will:

Develop individualized family service plan including identification of existing barriers to self-sufficiency, setting housing and income goals and developing an action plan.

Receive referrals to appropriate providers and resources.

Meet on a regularly scheduled basis to discuss, assess and receive direction and assistance in the removal of barriers to self-sufficiency.
